2 Months Hepatitis B, Rotavirus, DTaP, Haemophilus influenzae type b (Hib), Pneumococcal, Poliovirus
4 Months Rotavirus, DTaP, Haemophilus influenzae type b (Hib), Pneumococcal, Poliovirus
6 Months Hepatitis B, Rotavirus, DTaP, Haemophilus influenzae type b (Hib), Pneumococcal, Poliovirus
12 Months Haemophilus influenzae type b (Hib), Pneumococcal, Measles-Mumps-Rubella (MMR), Varicella*, Hepatitis A
15-18 Months DTaP
18 Months

Hepatitis A

4-6 Years DTaP, Pneumococcal, Measles-Mumps-Rubella, Varicella*

 * Varicella vaccine not required if child has ever contracted Chickenpox

11-12 years Tdap, Human Papillomavirus (3 doses for females), Meningococcal
